Your first visit

Before you meet us and to maximize the benefit of your initial appointment, we would like you to complete a full medical and dental history questionnaire. Please click on the download links to the left and fill out the appropriate forms prior to your dental appointment:

Part 1: Medical and Dental History

At your first appointment, you’ll be able to discuss your concerns or hopes for your teeth. We will then thoroughly examine your mouth, including:

• Facial aesthetic analysis
• Head and jaw-joint examination
• Oral soft tissue assessment including the floor of the mouth, tongue, pharynx, and oral cancer exam.
• A thorough soft-tissue (gums) assessment
• Complete tooth charting
• Full mouth 3D digital radiograph (Cone Beam Scan)
• Small digital radiographs, if required
• Intraoral and extraoral photographs

Dr Dunn will then discuss a detailed treatment plan. Your personalised treatment report will include the planned treatment and options, timing of appointments, and the fees involved.
